Braven Health PPO plans require PCP selection.

Agents must adhere to the required response time for CTMs/Complaints: within 24 hours of being sent [7 days/week, 24 hours/day].

For 2025, Braven Health Medicare Advantage Part D plans include a five-tier closed formulary.

The current requirement for Agents to record all calls with Beneficiaries includes only marketing, sales and enrollment calls.

Terminating Braven Choice PPO members will be cross walked to a new plan for 2025.
